• Attached below are the required vaccinations by the State of Texas for children attending public school.  


    For your child to be exempt from vaccinations you must complete the Immunization Exemption Form below and mail to the Austin address listed at the bottom of the form. Once the form is mailed, you will receive an affadavit that must be notarized and brought to the school to keep on file. You may make a copy for your records but the original is the only version that will be accepted by the school. Keep in mind this can take up to a month to complete so please be digilent in requesting and notarizing the affadavit before the start of school or before the previous affadavit expires. 


    Students who are non-compliant with these regulations will not be allowed to attend school until vaccinations are given or affadavit is completed.

    Vaccine Abbreviations and Names:

    • DTaP: Diphtheria, tetanus, and acellular pertussis (whooping cough); record may show DT or DTP
    • Polio: IPV - inactivated polio vaccine; OPV – oral polio vaccine
    • HepB: Hepatitis B vaccine
    • Hib: Haemophilus influenzae type b vaccine
    • PCV: Pneumococcal conjugate vaccine
    • MMR: Measles, mumps, and rubella vaccines combined
    • Varicella: Chickenpox vaccine. May be written VAR on record
    • HepA: Hepatitis A vaccine
