FOUR
YEAR OLDS (4) must have these immunizations to start to school:
DPT/DT/DtaP:Five (5) doses
(IF
DOSE 4 WAS RECEIVED ON OR AFTER 4TH BIRTHDAY THIS IS SUFFICIENT)
POLIO: THREE (3) doses
MMR: ONE (1) dose
HIB: ONE (1) dose on or after 15 months unless a primary series (3)
and booster has been completed.
HEP-B: THREE (3) doses. Takes 6 months to complete.
CHICKEN POX: (1) dose or verification*.
FIVE
to SIX YEAR OLDS (5-6) must have at least:
DPT/DT/DtaP: Five (5) doses including one after the fourth birthday.

SEVEN
YEAR OLDS (7) must have:
DPT/DT/DtaP: THREE (3) doses - One on or after the fourth birthday.
POLIO: THREE (3) doses - one on or after the fourth birthday.
MMR: ONE (1) dose on or after the first birthday.
MEASLES: ONE (1) dose - If born after Sept. 2, 1991. Usually given as a
second MMR.
*HEP-B: THREE (3) doses - If born on or after Sept. 2, 1992 (if not
previously completed).
CHICKEN POX: (1) dose or varification if born on or after Sept. 2,
1994.
TWELVE
YEAR OLDS (12) must receive all vaccines required of 7 year olds.
VARICELLA (chicken pox) - If born between September 2, 1988 and September
1, 1992 - must have one
dose or verification.
Hepatitis B: If born between September 2, 1988 and Septmeber 1, 1992 -
3 doses.
MEASLES: 2nd dose (which may be included in MMR) by twelth
birthday.
AGE
TWELVE (12) AND ABOVE must receive all vaccines required of 12 year olds
except those 18 and
above are not required to have POLIO.
DT: 10 year booster - Last bosster must be within 10 years.
THIRTEEN
YEAR OLDS (13) AND ABOVE must receive:
VARICELLA - 2 doses in required birthdate groups.
*Must be completed by age 5.
NOTES:
1.
Verification for chicken pox includes, parent, legal guardian, nurse or
physician and serologic confirmation.
2.
Parents of three year olds and younger children should check with the Collin
County Health Care at (972)548-5532 for requirements for preschool children.
